A Community Care Navigator from Halifax Health will be present in Halifax’s Community Care room in the Fins Up! Fitness Center.  The Community Care Navigator may provide the following services to residents, free of charge:

• Obtaining Blood Pressures

• Recording Blood Pressures for Resident to Submit to Care Provider

• Obtaining Height, Weight, & BMI (Body Mass Index)

• Identifying Resources for Nutrition Education

• Providing Guidance in Identifying Medical Resources in Community

• Providing Assistance in Scheduling Appointments with Physicians & Healthcare Centers

Please Note:  The Community Care Navigator cannot provide diagnosis or treatment; but can assist in identifying physicians, healthcare centers and other resources in the community for residents seeking diagnosis or treatment.
